Junior Active Leader Course – Pupil Resources

Please complete the requested information below to order the pupil certificates, pin badges and other resources. Please allow 3 weeks from the time you submit the form, for us to process and deliver your order.

You are asked to download and complete the spreadsheet with the names of the pupils you will train, AS YOU WOULD LIKE THEM TO APPEAR ON THEIR CERTIFICATES. Please use upper and lower case (eg Penny Snowden). This information will be imported onto the certificates. If you only wish to include first names you can leave the surname blank.

Each certificate will say:

If you have made an error after submitting the form and you need to change the details, please re-send the correct form to penny@activeleaders.co.uk as soon as possible. We reserve the right to re-charge if the certificates have already been printed, so please take care. Any re-prints will be charged at £5pp plus shipping.

If you have already submitted an order for pupil certificates and pin badges, but you would now like to place a second order for printed resources and t-shirts, please follow the instructions at the appropriate point on the form below.

Shipping has been calculated at second class postage. The contact named on this form will receive an e-mail when items have been despatched.

Please note that you have the option to ‘Save as Draft’ if you need others to provide you with required information (for example if another person needs to complete the Pupil Certification spreadsheet or provide the payment details). Please note that the ‘Save as Draft’ option will NOT save any uploaded files, so you need to wait until you are ready to make the payment and then upload the spreadsheet, enter the payment details and hit ‘Submit Resource Request’. The form will not allow you to submit, if you are trying to order certificates and pin badges, and you have not uploaded the required spreadsheet.

To complete this order form, the Pupil Certification list must be uploaded if you are ordering certificates and pin badges.
